A Personalised Care and Support Plan (PCSP) allows you to make some important choices for the care you receive before, during and after giving birth.
Your PCSP is based on your individual needs and what matters to you. You will have the chance to discuss these with your midwife or doctor and it will help to ensure that your personal choices are listened to.
Your PCSP will be either a booklet or an app on your phone, depending on where you are due to have your baby. You should bring your PCSP to every maternity appointment to support you with conversations about your care. Things which might be included in your PCSP include:
Your Personalised Care and Support Plan (PCSP) will help you explore, understand and record your individual choices for pregnancy, birth and the first weeks of parenthood.
The booklet contains QR codes to access further information. Simply hold your smartphone camera over any of the codes, and this will take you directly to the online information in more detail. You can translate the information into different languages using the 'Easy read and translate' tool on this website.
You should bring your PCSP to every maternity appointment to support you with conversations about your care;
